Medical awareness article explaining the connection between heart disease, blood clots and stroke, with a focus on prevention and early recognition of symptoms.

Cardiovascular Problems and Stroke: Understanding the Hidden Link

World Stroke Day 2026 | 29 October 2026

By Dr Asadullah Khan Soomro, Consultant Cardiologist and Heart Failure Specialist

A stroke affects the brain, but in some cases, the underlying danger begins in the heart. Blood clots that form inside the heart can travel through the bloodstream and block an artery supplying the brain, causing a cardioembolic stroke. Identifying and treating certain heart conditions can help prevent these potentially devastating events.

On World Stroke Day, observed on 29 October, understanding the connection between cardiovascular disease and stroke is essential for improving prevention, recognising warning signs and ensuring timely treatment.

Atrial fibrillation: An important risk factor

Atrial fibrillation (AF) is an irregular heart rhythm that can increase the risk of stroke. During AF, blood may pool in the upper chambers of the heart, allowing clots to form. If a clot travels to the brain, it can block blood flow and cause a stroke.

AF does not always produce noticeable symptoms. An irregular pulse should therefore be assessed by a healthcare professional. Doctors may recommend an electrocardiogram (ECG) or longer periods of heart-rhythm monitoring, depending on the individual’s condition.

Heart valve disease and stroke risk

Rheumatic heart disease, particularly narrowing of the mitral valve, can increase the risk of blood clots, especially when accompanied by AF.

People with artificial heart valves also require regular medical follow-up. Some patients, particularly those with mechanical valves, need lifelong treatment with warfarin to reduce the risk of clot formation. Regular blood tests help doctors monitor treatment and adjust doses when necessary.

Weak heart muscle and previous heart attacks

A severely weakened heart may allow blood to pool inside its chambers, increasing the possibility of clot formation. This risk can be particularly relevant after a major heart attack.

However, not everyone living with heart failure needs anticoagulant medication. Doctors determine the appropriate treatment based on the underlying condition, clinical findings and individual risk factors.

Other heart conditions that may cause stroke

Certain heart infections, including infections affecting the heart valves, can send infected material into the bloodstream and potentially to the brain.

Some congenital heart defects may also be associated with stroke. When a younger person experiences a stroke, doctors may investigate possible underlying causes, including heart conditions, where clinically appropriate.

Recognising stroke warning signs

Recognising stroke symptoms and seeking emergency treatment immediately can make a critical difference. Warning signs include:

  • Sudden drooping on one side of the face.
  • Sudden weakness or numbness in an arm or leg, particularly on one side.
  • Difficulty speaking or understanding speech.
  • Sudden loss of vision.
  • Sudden difficulty walking, dizziness or loss of balance.
  • A severe, unexplained headache.

If stroke is suspected, call emergency services and get the person to a hospital immediately—even if the symptoms disappear. Note the time the person was last known to be well, as this can help doctors determine appropriate treatment.

Do not give aspirin, food or drink while waiting for medical assessment. Some strokes result from bleeding in the brain, and aspirin may worsen the bleeding.

Warfarin and INR monitoring

Patients prescribed warfarin must take it exactly as directed and attend regular international normalised ratio (INR) blood tests. These tests help clinicians determine whether blood clotting is within the appropriate range.

The target INR varies according to the patient’s condition. For some people with mechanical heart valves, the prescribed range may be 2.5–3.5, although other targets apply to different patients and valve types.

Patients should never stop warfarin or change their dose without medical advice. If bleeding occurs, they should seek prompt medical guidance. Significant or persistent bleeding requires emergency assessment. Patients should also ask their treating team what to do about their next dose if bleeding or another complication develops.

Protecting the heart and brain

Reducing cardiovascular risk can also help lower the risk of stroke. Important preventive measures include controlling blood pressure and diabetes, avoiding tobacco, attending follow-up appointments and taking prescribed medicines consistently.

Following a stroke, doctors may use an ECG, an echocardiogram and other investigations to identify its cause. Treatment and prevention strategies should be tailored to each patient rather than applied uniformly.

A developing area of research: Atrial cardiomyopathy

Researchers are also studying atrial cardiomyopathy, a disease affecting the upper chambers of the heart, as a possible contributor to stroke—even in some people without documented atrial fibrillation.

Although this area of research may improve understanding of stroke risk, its clinical implications and the most appropriate preventive treatments remain under investigation.

Conclusion

The relationship between cardiovascular disease and stroke highlights the importance of early diagnosis, appropriate treatment and preventive care. Recognising irregular heart rhythms, managing heart valve disease and controlling cardiovascular risk factors can help reduce the risk of certain strokes.

On World Stroke Day 2026, the message is clear: protecting the heart can help protect the brain. Anyone experiencing possible stroke symptoms needs immediate emergency medical attention.
